The Diabetes Center on the campus of Deborah Heart and Lung Center provides diabetes treatment and education. An integrated team of specially trained physicians, nurses, and dietitians creates an individualized care plan with the patient and their family, ensuring positive outcomes.
Patients are assessed and evaluated for the type of diabetes and its severity, and a treatment plan is created with appropriate therapies and medication. A crucial component to diabetes self-management is education. The Deborah Specialty Physicians team includes Certified Diabetes Educators who instruct patients in understanding and managing their diabetes. These Educators help patients develop problem-solving and coping skills – such as how to order at restaurants and how to read labels in the supermarket – that allow patients to make the best decisions for their health and well-being.
In addition to education, patients are provided with customized meal plans and follow-up sessions with a dietitian to evaluate the meal plan’s effectiveness. Working with the diabetes team, patients are guided into active self-management that improves quality of life.
